Wraparound is a team-based, family driven, and youth-guided planning process used to address complex needs for your child and family.
Wraparound creates relationships by bringing a team of involved people together to support you, your child, and your family. Team members work together to create a system of supports that help our family move forward with confidence.
A free and voluntary program helping families solve difficult issues. The referred child must be between the ages of 0-21. Meetings can take place in the home, at school, in-office, or online and the process can last up to 12-14 months as strengths are built and needs are met.
A family can be referred to the program by anyone (ex: self-referral, schools, Juvenile Assessment Center, or any number of other agencies).
A Wraparound care coordinator meets with the family to explain the process and highlight the voluntary and family-friendly nature of the process. The facilitator listens to the family’s take on their situation and conducts a strengths-based assessment. Wraparound is based on family voice and choice.
Based on the needs and strengths assessment, the family and facilitator will identify people to be invited to be a part of their Wraparound team and attend meetings or be updated on the care plan; the majority of people invited to participate in the meetings are the family’s natural resources, such as teachers, pastors, friends, relatives, doctors or whomever the family & youth choose as supports. The family often also invites agency representatives or professionals who could potentially lend expertise or services to address some of the needs identified in the assessment such as counselors or probation officers.
The Wraparound team, including the family, meets to discuss needs, strengths, and strategies to address the issues and build on strengths. The care coordinator guides the process of delineating clear action items for which members of the team accept responsibility.
The team operates in an “anything is possible” atmosphere, examining the differing factors in the family’s situation and are incredibly creative in addressing those factors and utilizing team resources.
With support from the Wraparound team, the family implements the action plan. The facilitator continues to provide advocacy and support services, but team members are the doers in this process. The idea is to establish sustainable supports that won’t just disappear after Wraparound team meetings end.
